- Memorandum of Agreement Affidavit or consent of property owner (notarized) and any Government ID from the property owner
- Urban Poors Association Office (UPAO) Certification (if applicable)
- Location Plan or sketch of the location clearly showing where business premises is located
- Clearances or Certificate from Barangay, Zoning/MPDO, Engineering Department, MENRO, Sanitary Unit, PESO, Tourism Office, DPOS, MDRRMO, Bureau of Fire Protection and Treasury Office (Market Clearance if applicable)
- Clearances/Certificate/Accreditation from National Agencies (if applicable)
- Affidavit of Undertaking (if applicable)
- Special Power of Attorney of the authorized immediate family representative and Photocopy of valid ID of the owner
- Audited Financial Statements (AFS) o unaudited AFS para sa mga hindi kinakailangang mag-file ng AFS sa BIR
- Sworn declaration of Gross Sales or Receipts
- Income Tax Return
- Clearances Certificate Accreditation from National Local Government
- Agencies (depending on the type of business).
- Copy of Guest List/Log Book with name, age, address, contact number, date of check in, date of check in, days of stay and copy of valid ID with picture of guest (For owners of transient apartment or hotel)
- Renter's ID and Contract of Lease (For owners of apartment, townhouses, bedspace, residential, house, rooms rent, boarding)
- Lessor's Mayor's Permit (if the applicant is a lessee)
- Affidavit of Undertaking (if applicable)
- Special Power of Attorney of authorized representative and any Government-issued ID of owner Additional Requirements from Bureau of Fire Protection (BFP):
- Picture of business establishment
- Sketch of location
- Previous Fire Safety Inspection Certificate (FSIC)
- Fire Safety Maintenance Report (Photocopy of official receipt (O.R.) of Fire Extinguisher)
